Homebound Vaccinations
Middlesex County is committed to vaccinating our older and disabled residents who may not be able to visit a County vaccination location. If you are a homebound individual and have not yet received a COVID-19 vaccine, please call our COVID-19 Call Center at 732-745-3100 to speak directly with our staff where you can arrange for a nurse to administer the vaccine to you at home.
Medicaid members can schedule transportation to a vaccine appointment by calling Modivcare at 1-866-527-9933 at least 48 hours before the appointment.
